Just fold the pocket fabric in half and line up the folded edge with the top line of the pocket placement.
It is simply, just like a usual applique design.

If you need a pocket to be usable, but with a monogram on it, embroider it before attaching the pocket to the item.

Stitch the placement line, place a piece of appliqué fabric over it, then stitch the tack-down line. Trim the excess fabric close to the seam, and finish with the cover stitches.
